- Download the notification and the application form (Annexure) from esic.gov.in/recruitments.
- Fill a separate form for each post you want, in capital letters, and paste a recent passport-size photograph on it.
- Attach two self-attested photographs and self-attested copies of your date-of-birth proof, degrees, registration, experience certificates and publications, plus your category certificate and NOC if applicable.
- Report at ESIC MCH, Desula, MIA, Alwar (Raj.) 301030 between 9:00 AM and 10:30 AM on your department’s date with all originals and the Rs. 500 fee if it applies to you.
- Super Specialists: send the Annexure ‘A’ form with documents in a sealed envelope by Speed Post, Registered Post or by hand to The Dean, ESIC Medical College and Hospital, Alwar – 301030, to reach by 12:00 PM on 30.09.2026.
