Selection Process

  3. Type the form neatly. Handwritten forms are not accepted, and no column may be left blank. Stick a recent colour passport photo (7 cm tall, 5 cm wide) in the box and sign below it in blue ink.
  4. Attach copies of your MBBS and MD or DNB certificates and mark lists, proof of the other details you gave, and the fee receipt.
  5. Put everything in a sealed envelope marked “APPLICATION TO THE POST OF MEDICAL OFFICER – SD, POST CODE NO:” and write 01, 02 or 03 after it. Send it by ordinary post so that it reaches HSFC by 08.11.2026.

Common Mistakes to Avoid

  • Do not use courier, hand delivery or email. The notification says only ordinary post is accepted and anything else will not be considered.
  • Do not post it on the last day. The application must reach HSFC by 08.11.2026, and HSFC is not responsible for postal delays or lost post.
  • Do not send a handwritten form or leave a column blank. Incomplete applications are rejected.
  • Check the photo and signature rules: colour photo under 6 months old, plain white background, no selfie, blue ink. Breaking any one of them cancels your candidature.
